Tax Return Forms for 1040 Document Request

1040 Forms & Examples:

When you submit your most recent 1040 tax return, please also include any of the following schedules or forms that are part of your return, since they contain information we need beyond what's on the main 1040 page:

  • Schedule 1 — Additional income (like unemployment, business income, or other taxable income) and adjustments to income

  • Schedule 2 — Additional taxes (like self-employment tax or alternative minimum tax)

  • Schedule 3 — Additional credits and payments

  • Schedule C — Profit or loss from a sole-proprietorship business

  • Schedule E — Income from rental real estate, royalties, partnerships, S-corporations, trusts, and estates

  • Schedule F — Profit or loss from farming

  • Schedule K-1 — Your share of income from a partnership, S-corporation, trust, or estate (you'll get one of these for each business you have an ownership stake in)

If you're not sure which of these apply to you, check page 2 of your 1040 and any accompanying schedules for your name — if a schedule was filed as part of your return, you'll typically see it listed in the "Attachments" or included directly behind your 1040 in the PDF you download from your tax preparer or software.
